Before starting Vivitrol, you must be completely opioid-free for at least 7–10 days to avoid precipitated withdrawal (a sudden and severe withdrawal reaction).
✔ Opioid Use Disorder (OUD): You must complete detox and be off opioids (including Suboxone, methadone, heroin, fentanyl) for at least 7–10 days.
✔ Alcohol Use Disorder (AUD): No detox required, but alcohol cravings should be managed before starting.
Your doctor may:
✅ Perform urine drug tests to confirm you are opioid-free.
✅ Offer medications or support to help manage withdrawal before starting Vivitrol.
✅ Discuss your treatment goals and potential side effects.
✔ Once you are opioid-free, your doctor will give the first Vivitrol injection in the buttock muscle (intramuscular).
✔ The injection slowly releases naltrexone over one month, continuously blocking opioid effects and reducing alcohol cravings.
